Perioperative antibiotic prophylaxis should be administered within 60 minutes before incision, ideally at the time of induction of anaesthesia.
A single dose of perioperative antibiotic prophylaxis is preferred, but redosing is required for an operative duration of >4 hours (cefazolin) or >2 hours (piperacillin/tazobactam) and for blood loss of >1500 ml.
