In patients with a normal course of symptoms, antimicrobial therapy is not indicated.
Consider antimicrobial therapy in patients with an (increased risk of an) atypical course: in those with reduced immune function, fever > 5 days, or recurrence of fever after a number of afebrile days.
In chronic sinusitis, treat only during exacerbations guided by the susceptibility pattern.
