+ amikacin IV 20 mg/kg once daily+ imipenem + cilastatin IV 500 mg four times daily |
Initial treatment duration 2-6 weeks, depending on response. Final duration of therapy 9-12 months guided by species identification and susceptibility testing. Cotrimoxazole dose for weight < 50 kg: 1440 mg IV three times daily. |
