Consider prophylactic antibiotics in patients in risk groups: - Increased risk of a complicated course due to reduced immunity: including untreated HIV infection, previous transplantation (organ/stem cell/bone marrow), (haematological) malignancy, asplenia, multiple sclerosis, medications (immunosuppressants, cytostatics (sometimes up to 1 year after use)). - Increased risk of endocarditis: history of endocarditis, prosthetic heart valves or congenital valvular heart disease. - Increased risk of prosthetic joint infection: a joint prosthesis < 2 years old, a joint prosthesis > 2 years old in combination with a history of an infected prosthesis, rheumatic joint disease or haemophilia.
Advice:
First choice: flucloxacillin 500 mg 4 times daily for 7 days For penicillin allergy: clindamycin 600 mg three times daily for 7 days
No prophylaxis.
