Biguanide monotherapy (chlorhexidine 0.02% or PHMB 0.02% or 0.08%), with or without propamidine (Brolene) 0.1%:
24 times daily for 2 days;
Then 16 times daily for 5 days;
Then 8 times daily for 7 days;
Then 6 times daily for at least 7 days.
Availability of polyhexamethylene biguanide (PHMB) and propamidine isethionate (Brolene®) in the Netherlands is limited. A physician's declaration is required for use of propamidine isethionate.
Gradually taper medication further based on the clinical picture to 4 times daily. Continue this dosing for at least 8 weeks up to 12 months until clinical cure.
