Treatment with permethrin and ivermectin are equivalent.
Permethrin for frail elderly patients, immunocompromised patients and in scabies crustosa: also treat the face, neck, scalp and ears.
Permethrin kills mites and eggs; it should be reapplied after using the toilet, and after changing sanitary pads and (incontinence) diapers.
Children >12 years and adults 1 tube of 30 grams;
6 -12 years 1/2 tube;
3- 5 years 1/4 tube;
1 - 3 years 1/4 tube;
2 months - 1 year 1/8 tube
Ivermectin on an empty stomach; do not eat for 2 hours after taking ivermectin. Maximum 18 mg per dose.
In outbreaks of scabies in large groups, ivermectin is used as the first choice.
Ivermectin should not be given to infants up to 1 month or during pregnancy, although harmful effects have not been demonstrated. Ivermectin IS permitted during breastfeeding for infants older than 7 days. For very young children (< 15 kg) there is no suitable formulation available on the market.
The use of permethrin is also permitted during pregnancy and lactation. During pregnancy and lactation, benzyl benzoate may (as a second choice) also be used, benzyl benzoate ointment 25% FNA. After application of benzyl benzoate the area must not be washed for the 48 hours the treatment takes. After the first 24 hours reapply to the entire body according to the above instruction. Only 24 hours after the second treatment should the whole body be washed with lukewarm water and soap. Repeat the treatment after 7 days again according to the above method. Benzyl benzoate does not kill the eggs.
