Step 1

• Miconazole cream 20 mg/g or ketoconazole cream 20 mg/g, 1–2 times daily, or terbinafine cream 10 mg/g, once daily, applied to the affected areas and a 2 cm margin for 2 weeks 
• OR selenium sulfide shampoo 2.5% to the affected areas, once daily for 1 week; rinse off thoroughly after 10 minutes' contact.

Step 2

If response is insufficient, switch to one of the other methods (topical imidazole, terbinafine or selenium sulfide).

Step 3 

Itraconazole orally 200 mg once daily for 1 week.
