Recommendations

≥ 18 years

Indications: Katayama syndrome
Dosage Remarks
praziquantel orally 40 mg/kg once daily for 3 days

If necessary, also give corticosteroids for 3 days

Repeat praziquantel after 3 months
≥ 18 years

Indications: S. mansoni, S. haematobium, S. intercalatum
Dosage Remarks
praziquantel orally 40 mg/kg single dose

Repeat after 4 weeks
≥ 18 years

Indications: S. japonicum, S. mekongi
Dosage Remarks
praziquantel orally 30 mg/kg twice daily for 1 day

Repeat after 4 weeks
≥ 18 years

Indications: CNS localisation
Dosage Remarks
praziquantel orally 30 mg/kg twice daily for 3 days

Repeat after 4 weeks

Comments

Katayama syndrome; praziquantel is not effective against young parasitic stages (schistosomula). Therefore always repeat praziquantel treatment once after 3-6 months.

Therapeutic failure after a single dose of praziquantel occurs frequently (20-30%). Repeat treatment after 4 weeks.

CNS localisation: praziquantel with high-dose corticosteroids (prednisone 60 mg/day) for 28 days.
