amoxicillin + clavulanic acid oral 875/125 mg three times daily for 7 days

doxycycline oral loading dose 200 mg, 100 mg once daily for 7 days

Indications: Beta-lactam allergy

clarithromycin oral 500 mg twice daily for 7 days

azithromycin oral 500 mg once daily for 3 days

Comments

H. influenzae: resistance to amoxicillin: 18% in hospital, 15% in general practice; co-trimoxazole: 19% in hospital, 15% in general practice;  amoxicillin-clavulanic acid: 7% in hospital, 9% in general practice;  doxycycline 3% in hospital, 1% in general practice. 

 

NHG COPD Guideline 2015:

In patients with ≥ 1 criterion for a severe exacerbation who can be treated in the home setting:

· In all cases: a course of oral corticosteroids

· Antibiotics are indicated depending on FEV1 and clinical signs of infection; with FEV1 <30% antibiotics are always indicated.
