Clindamycin: in patients with a body weight > 180 kg increase the clindamycin dose from 600 mg to 900 mg. Metronidazole: the advice is not to adjust the metronidazole dose in overweight patients. Vancomycin: in patients with a total body weight above 100 kg dose vancomycin as 10 mg/kg total body weight. Cefuroxime: For children older than 1 month, a dose of 50 mg/kg applies (maximum 1.5 grams). Re-dosing is required if the duration of the operation exceeds 140 minutes.
